Colors of You Counseling and Consulting PLLC is an in-network provider for clients with most Aetna and Blue Cross Blue Shield plans (except Blue Home and Blue Local). You are responsible for understanding your insurance plan, your deductible, and your copay or coinsurance, as well as updating me if any of your insurance information changes. Often exact benefits cannot be determined until an insurance claim is processed. You are responsible for any charges that insurance does not cover.
Please be aware that if you use insurance for sessions, a DSM-5 diagnosis must be reported to your insurance, and at times I may be required to share additional information with them.
If you want to better understand your benefits, I recommend calling your insurance’s member services number (usually listed on the back of your insurance card) and asking some of the following questions.
Is outpatient mental health therapy covered (CPT codes 90791 and 90837)?
Are telehealth visits covered?
Do I have a deductible (and what is the amount if so)? What is my coinsurance if I meet my deductible?
Do I have a copay (and what is the amount if so)?
Is Colors of You Counseling and Consulting PLLC in network with my specific plan?
Insurance
Payment and cancellation policy
Payment for sessions is due at the time of service, and it’s required to keep a functioning credit or debit card on file at all times. Payment information is securely stored within a HIPAA-compliant electronic health records system.
If you need to cancel a session, please reach out 24 hours in advance via email or text. If you cancel with less than 24 hours notice, you will be charged a $135 late cancel fee. If we’re able to reschedule within the same Monday to Friday week, I will waive the late cancel fee (otherwise the $135 late cancel fee will apply).
If you’re running late for a session, I will remain in the virtual waiting room for 15 minutes. If you haven’t arrived by 15 minutes into our scheduled session time, you will be charged a $135 no show fee.
Please note that if you no show 2 times in a 4 week period, you will be discharged from therapy. It’s also important to be aware that insurance does not cover no show or late cancel fees.
Sessions are scheduled at the end of our appointments and/or through email or text. If we have not had a session in 4 weeks and you have not responded to my attempts to reach you about rescheduling, you will be discharged from therapy. This does not apply if you communicate with me about 4+ week periods you will be unable to meet, and we create a plan for when we will reconnect.
Under the law, health care providers need to give individuals who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
